About the Role

We're hiring a Remote Bilingual Care Coordinator to join our innovative team at Pair Team. As a Remote Bilingual Care Coordinator, you will play a crucial role in delivering compassionate, high-impact care to underserved populations. Our mission is to reimagine how Medicaid and Medicare serve these communities by providing whole-person care that includes clinical, behavioral, and social support.

What You'll Do

• Coordinate care for high-need patients, ensuring they receive timely and personalized support.
• Collaborate with a diverse team of nurses, social workers, and community health workers.
• Leverage AI and automation to streamline administrative processes and reduce burdens on care teams.
• Engage with patients and their families to provide education and support regarding their care plans.
• Monitor patient progress and adjust care plans as necessary to improve outcomes.

Requirements

• 2-5 years of experience in care coordination or a related field.
• Fluency in English and at least one other language.
• Strong communication and interpersonal skills.
• Experience working with underserved populations is a plus.
• Ability to work collaboratively in a remote team environment.

• Experience with AI and technology in healthcare.
