About the Role

We are seeking a dedicated Remote Patient Monitoring Coordinator to join our team on a part-time basis during weekends. In this role, you will play a crucial part in ensuring effective patient engagement and monitoring, contributing to the overall quality of care provided to our patients. As a Remote Patient Monitoring Coordinator, you will work closely with healthcare professionals to manage patient data and facilitate communication between patients and medical staff.

What You'll Do

• Coordinate patient monitoring activities, ensuring timely and accurate data collection.
• Engage with patients to provide support and guidance on their health management plans.
• Document patient interactions and maintain accurate records in our electronic medical records (EMR) system.
• Assist in scheduling appointments and follow-ups, ensuring a seamless patient experience.
• Collaborate with healthcare teams to analyze patient data and improve care plans.

Requirements

• Experience in patient coordination or a related field is preferred.
• Familiarity with EMR systems and data management practices.
• Strong communication skills to effectively engage with patients and healthcare professionals.
• Ability to work independently and manage time effectively in a remote setting.
• Basic knowledge of medical terminology and patient care processes.
