About the Role

We are seeking a Director of Post Acute Care to join our innovative team at Navvis, a leading population health company. This remote position allows you to drive performance in value-based care while collaborating with some of the country’s most progressive health systems. You will play a crucial role in enhancing healthcare delivery and ensuring that patients receive the quality care they deserve.

What You'll Do

  • Implement and monitor data-driven performance dashboards for Post-Acute Alliance reporting.
  • Regularly review and report on the performance of Post-Acute Care Alliance providers through quarterly performance scorecards.
  • Identify performance gaps and develop corrective action strategies to progress toward goals.
  • Provide regular report-outs to internal and external teams to enhance visibility into post-acute performance.
  • Conduct sessions with skilled nursing facilities to maintain communication and confirm progress toward shared goals.
  • Lead a team of Post-Acute Care associates in delivering high-performance case management.
  • Ensure collaboration between PAC associates and key client partners to streamline processes and optimize care delivery.
  • Review performance metrics of non-alliance agencies to evaluate performance opportunities.

Requirements

  • Valid clinical licensure for Massachusetts in RN, SW, PT, or OT.
  • 5 years of strategy, operational, or clinical leadership in the post-acute industry.
  • Experience in operations or leadership within Skilled Nursing Facilities, Inpatient Rehabilitation, Home Health, and/or Hospice.
  • Proven experience in creating and executing data-driven performance improvements.
  • Strong leadership, presentation, and communication skills.
  • Experience with Microsoft Office and virtual conferencing platforms.
  • Must be located in the Boston, MA area and willing to travel up to 80%.
