Care Coordinator - Remote Opportunity

About the Role

We are excited to announce a remote opportunity for a Care Coordinator to join our team at Routes Healthcare. As a Care Coordinator, you will play a vital role in scheduling and coordinating care visits to ensure our clients receive top-notch care when they need it. This position offers a competitive salary of up to £27,500, including on-call payments, plus bonuses based on service performance.

What You'll Do

• Coordinate the delivery of care using our care scheduling system.
• Adapt to changes in schedules, including staff availability and new clients.
• Ensure healthcare workers attend staff meetings, supervisions, and training within CQC-required timescales.
• Assist your Registered Manager in delivering high-quality care services.
• Manage on-call duties on a rotating basis.

Requirements

• Passion for putting people first and delivering exceptional care.
• Strong organizational and time management skills.
• Understanding of CQC regulations.
• Excellent communication skills, with the ability to coordinate a team of healthcare workers.
• An NVQ in Health and Social Care is a significant advantage.
• Full valid UK driving license and access to your own vehicle.

Nice to Have

• Experience in a similar role within the healthcare sector.
• Familiarity with digital care planning systems.
• Previous experience in team coordination or management.
