• Assess and review individual care plans, ensuring they meet the needs of our clients.

• Organize rotas to impeccable standards, ensuring efficient service delivery.
• Build strong relationships with care workers and clients to enhance service quality.

Requirements

• Proven experience as a Care Coordinator or similar role in healthcare.
• Strong organizational skills and the ability to manage multiple tasks effectively.
• Valid driver’s license and access to your own vehicle due to travel requirements.
• Understanding of CQC standards, care plans, and rotas.
• NVQ Level 3 in Health and Social Care is desirable.
