About the Role
We're hiring a Senior Social Worker - Diabetes Education to join our team at Goulburn Valley Health. This part-time remote role offers an exciting opportunity to enhance the multidisciplinary capacity of our Diabetes Centre, focusing on improving client outcomes for individuals diagnosed with Diabetes Mellitus.
What You'll Do
- Provide psychosocial assessments and client advocacy for both pediatric and adult clients.
- Deliver education and counseling related to diabetes self-management.
- Assist clients in meeting their identified needs through referrals to other agencies.
- Focus on continuous improvement and evaluate the quality and outcomes of social work interventions.
- Collaborate with a multidisciplinary team to enhance service delivery.
Requirements
- A degree in Social Work and relevant experience in diabetes education.
- Proven ability to work effectively in a multidisciplinary team.
- Strong communication and interpersonal skills.
- Experience in providing client-centered care and advocacy.
- Ability to work independently and manage time effectively.
