RN Case Manager - Home Health (Remote)
About the Role
We are seeking a dedicated RN Case Manager to join our team remotely. In this role, you will oversee the delivery of coordinated care for patients, ensuring they receive the highest quality of home health services. As an RN Case Manager, you will play a crucial role in managing clinical care activities and enhancing patient outcomes.
What You'll Do
- Oversee the delivery of coordinated care for patients, ensuring compliance with care plans.
- Conduct in-home visits and attend physician appointments to provide direct patient care.
- Document patient interactions and care plans accurately and timely to facilitate reauthorization.
- Assist in the orientation and training of new staff members, ensuring they are well-prepared for their roles.
- Collaborate with healthcare teams, including physicians and payers, to promote optimal patient outcomes.
- Participate in quality assurance committees and contribute to process improvement initiatives.
- Maintain up-to-date knowledge of payer policies and assist with state survey preparations.
- Perform supervisory visits as needed and provide guidance to nursing staff.
Requirements
- Associate's or Bachelor’s degree in a related field.
- Current Registered Nurse (RN) license in the state of practice.
- Basic Cardiac Life Support certification.
- Minimum of one year of case management experience.
- Experience in home health care is strongly preferred.
Nice to Have
- Experience with Department of Labor (DOL) / EEOICPA regulations.
- Previous supervisory experience in a healthcare setting.
- Familiarity with electronic health record systems.
