About the Role

We are seeking a dedicated Clinic Social Worker to join our team in a remote capacity. As a Clinic Social Worker, you will provide immediate assessment and assistance for patients, playing a crucial role in managing crises and determining appropriate treatment paths. This position is vital for ensuring the well-being of our patients and their families.

What You'll Do

  • Evaluate patients through interviews and gather necessary clinic, social, and health information for effective analysis.
  • Advise patients about their rights, responsibilities, treatment consents, and proper actions.
  • Analyze, plan, develop, and administer patient treatments focusing on their well-being and participation.
  • Offer individual, family, and group therapy, ensuring actions benefit the patient.
  • Revise and monitor treatment plans, consulting with other professionals to assure patient well-being.
  • Refer and coordinate with community agencies for additional services beneficial to treatment.
  • Document attendance and interventions in patient files, maintaining confidentiality.
  • Comply with all guidelines established by regulatory agencies, including the Centers for Medicare and Medicaid (CMS).

Requirements

  • Master’s degree in Clinical Social Work.
  • Current, unrestricted license to practice in Puerto Rico and Professional College Membership.
  • Minimum of 2+ years of experience in the mental health field.
  • Extensive knowledge of mental health laws and regulations.
  • Proficiency in Microsoft Word, Excel, PowerPoint, and Outlook.

Nice to Have

  • Experience working with diverse populations, including children and individuals with disabilities.
  • Familiarity with DSMV, diagnosis, and pharmacology.
  • Strong communication and interpersonal skills.
