What You'll Do
- Collaborate with the patient’s Primary Care Provider (PCP), Psychiatric Consultant, and the broader interdisciplinary care team to deliver integrated, evidence-based behavioral health services to pediatric patients experiencing conditions such as ADHD, anxiety, and depression.
- Coordinate and manage a defined caseload, ensuring seamless integration of mental and physical healthcare services.
- Support the mental health care of patients on an assigned patient caseload under the guidance of the patient’s primary care provider.
- Screen and assess patients for common pediatric mental health symptoms and disorders.
- Facilitate patient engagement and determine follow-up care according to acuity.
- Provide patient education about common pediatric mental health disorders and the available treatment options.
- Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
- Support psychotropic medication management prescribed by medical providers by monitoring and encouraging adherence and assessing side effects and effectiveness of treatment.
Requirements
- Master’s degree in Social Work (MSW) from an accredited program required; preference given to candidates holding an active, unrestricted Licensed Clinical Social Worker (LCSW) credential in the State of Georgia.
- Minimum of one (1) year of post-graduate professional experience in behavioral health treatment, social services, or care management.
- Current and unrestricted licensure as a Licensed Clinical Social Worker (LCSW) in the State of Georgia.
- Demonstrated experience in pediatric behavioral health across outpatient, inpatient, or emergency department settings.
- Strong working knowledge of prevalent pediatric mental health and substance use conditions.
Nice to Have
- Experience utilizing digital patient registries or population health management tools to track outcomes and support measurement-based care.
- Proficiency with Epic electronic health record (EHR) systems.
- Experience working within the Collaborative Care Model (CoCM) or other integrated behavioral health frameworks within primary care settings.
