Senior Social Worker - Remote Opportunities Available
About the Role
We are seeking a dedicated Senior Social Worker to join our team at Educational Alliance. This Senior Social Worker remote position offers the opportunity to work with older adults in a supportive and engaging environment. You will be responsible for maintaining a caseload of clients, providing comprehensive assessments, and ensuring that they receive the necessary resources and support to thrive.
What You'll Do
- Maintain a caseload of older adult clients, providing comprehensive assessment of needs, case management, supportive counselling, and routine follow-up.
- Formulate ongoing care plans based on clients' challenges, needs, and strengths, and make referrals to external agencies and appropriate services.
- Connect clients to applicable resources and intervention strategies, monitoring and evaluating the effectiveness of interventions.
- Work collaboratively with on-site nurses and interdisciplinary teams to ensure consistency of service to clients.
- Oversee the Senior Companions Friendly Visiting Program and facilitate group activities.
- Participate in biweekly case conferences and attend supervisory, agency, and community meetings as required.
- Maintain up-to-date case records and program statistics in the documentation database.
- Assist with organizing NORC and OAS events as needed.
Requirements
- Must possess an LMSW and have a minimum of 1 year of experience working with older adults and/or in social services.
- Knowledge of resources and entitlements, as well as age-related cognitive and mental impairments, is required.
- Strong attention to detail and the ability to work under pressure while coordinating a comprehensive array of services.
- Ability to take initiative, work independently, and prioritize tasks effectively.
- Strong verbal and written communication skills are essential.
- Proficiency in MS Office (Outlook, Word, Excel) is required.
Nice to Have
- Experience in community engagement and program development.
- Familiarity with grant writing and fundraising initiatives.
- Previous experience in a leadership role within social services.
