We're hiring a Remote Nurse Practitioner to join our dedicated team at Ennoble Care. This role is vital in providing high-quality primary care to homebound and palliative patients in the Mercer County, NJ region. As a Remote Nurse Practitioner, you will apply your medical expertise to ensure that our patients receive the best possible care in the comfort of their homes.

What You'll Do

  • Provide high-quality care to primary and palliative patients in and around Mercer County, NJ.
  • Assess patients' health by performing physical examinations, obtaining, updating, and studying medical histories, and creating the best plan for care.
  • Effectively communicate with office staff and clinical operations across various internal platforms.
  • Attend quarterly medical staff meetings to discuss patient care and operational improvements.
  • Develop impressions, diagnoses, and treatment plans tailored to individual patient needs.
  • Foster trust and consult with patients, families, and caregivers during visits.
  • Order diagnostic testing as medically necessary to ensure comprehensive patient care.

Requirements

  • Must be a licensed Nurse Practitioner with experience in primary care or palliative care.
  • Strong communication skills and the ability to work collaboratively with a healthcare team.
  • Experience with remote patient monitoring and chronic care management is preferred.
  • Ability to assess and manage a diverse set of patients with chronic diseases.
  • Commitment to providing patient-centered care and fostering trust with patients and their families.
