Remote Care Specialist - Home Health Services

About the Role

We are seeking a dedicated Remote Care Specialist to join our team at Evangelische Stiftung Neuerkerode. In this role, you will provide essential support to individuals in need of home health services, ensuring they receive the highest quality of care. As a Remote Care Specialist, you will have the opportunity to make a meaningful impact in the lives of those you serve.

What You'll Do

• Deliver basic and treatment care to clients, addressing their individual needs.
• Manage digital documentation and quality assurance processes to uphold care standards.
• Provide guidance and support to clients and their families on health promotion and care organization.
• Actively participate in the development and implementation of internal quality management systems.
• Collaborate with a multidisciplinary team to enhance service delivery and client satisfaction.

Requirements

• Completed training as a certified elder care nurse, health and medical nurse, or care specialist.
• Class B driver's license.
• Strong independence and communication skills.
• Empathy and a genuine passion for working with individuals in need of assistance.
• Experience in home health care is a plus.

Nice to Have

• Additional certifications in health promotion or care management.
• Experience with digital documentation tools.
• Knowledge of quality management systems in healthcare.
