Registered Nurse - Community Care (Remote)
About the Role
Valor Healthcare is looking for a passionate Registered Nurse to join our team at the Community Based Outpatient Clinic (CBOC) in Kingston, New York. As a Registered Nurse in this remote position, you will assist practitioners with patient care, physical examinations, and ancillary tests. What You'll Do
- Actively assist practitioners with patient care, physical examinations, and ancillary tests.
- Practice specialized skills in patient assessment and triage unscheduled patients requesting care.
- Serve as the point person within the PACT Team, collaborating with family/caregiver, VA, and community-based services to develop patient care plans.
- Provide patient and family health education with a focus on self-management, prevention, and wellness.
- Monitor the VA discharge list and contact patients for follow-up.
- Maintain confidentiality of veterans’ data and comply with all VA and company training requirements.
- Consult as permitted by VAMC and fulfill compliance requirements of the Office of Inspector General (OIG) and Joint Commission (JC).
- Participate in the clinic’s outreach events to support enrollment initiatives.
Requirements
- Bachelor of Science in Nursing (BSN) required; if VA contract allows, may be Associate Degree RN.
- Minimum 2-3 years’ experience as a registered nurse in a related primary care or ambulatory care setting; government healthcare environment preferred.
- Must be credentialed and remain in good standing through the Veterans Health Administration (VA).
- Current certification in Basic Life Support and ACLS as specified by individual VAMC contract.
- Proficiency in written and spoken English with strong computer skills, including EMR experience.
- Energetic and optimistic demeanor with a strong service mentality.

Who Will Succeed Here
