Home Health Licensed Practical Nurse - Remote Opportunity

About the Role

We are seeking a dedicated Home Health Licensed Practical Nurse to join our team in providing exceptional in-home care to our patients. This Home Health Licensed Practical Nurse remote position allows you to deliver skilled nursing care ordered by physicians, ensuring that patients receive the best possible support in their own homes.

What You'll Do

• Provide in-home nursing services to patients, following the care plan established by the physician.
• Regularly review caseload with the supervising RN to ensure compliance with best practices and state guidelines.
• Obtain necessary physician orders and collaborate with healthcare providers based on patient needs.
• Evaluate assessment findings and communicate significant clinical information to the healthcare team promptly.
• Educate patients, caregivers, and families about care procedures and health management.
• Participate in case conferences to discuss patient progress and multidisciplinary responsibilities.
• Implement safety measures to reduce the risk of medical errors and ensure patient safety.
• Document visit notes accurately via Electronic Health Record (EHR) for every patient in a timely manner.

Requirements

• Valid LPN license and experience in home health care.
• Strong communication skills to effectively collaborate with healthcare teams.
• Ability to assess patient needs and provide appropriate care.
• Experience with Electronic Health Records (EHR) systems.
• Commitment to patient safety and quality improvement initiatives.
