About the Role

We are looking for a passionate Case Manager I - Remote to join our team at Patient Advocate Foundation (PAF). This role is crucial in helping patients navigate their healthcare challenges and ensuring they receive the support they need. As a Case Manager I, you will be the primary point of contact for patients with serious health conditions, advocating for their needs and helping them overcome barriers to care.

What You'll Do

  • Engage with patients to identify their healthcare and financial issues, developing tailored action plans to address their needs.
  • Utilize various IT platforms to manage patient cases effectively and respond to their changing needs.
  • Conduct thorough follow-ups with patients and agencies to ensure timely processing of requests and communication of outcomes.
  • Assist patients in securing financial assistance and navigating complex healthcare systems.
  • Provide education on workplace protections and available resources for patients facing financial hardships.

Requirements

  • 2+ years of experience in financial navigation, healthcare navigation, or social services.
  • Strong problem-solving skills and the ability to work independently in a fast-paced environment.
  • Excellent communication skills, both verbal and written, to effectively interact with patients and external stakeholders.
  • Experience in negotiating with landlords or utility companies on behalf of patients.
  • Familiarity with healthcare laws and programs such as COBRA, FMLA, and ADA.

Nice to Have

  • Experience working with patients in crisis or those facing serious health conditions.
  • Knowledge of community resources and financial assistance programs.
