What You'll Do

• Conduct in-home visits and telehealth consultations for patients with complex healthcare needs.
• Provide comprehensive assessments and develop individualized care plans.
• Collaborate with a team of healthcare professionals to ensure coordinated patient care.
• Document patient interactions and maintain accurate medical records.
• Participate in ongoing training and development to enhance your skills as a Nurse Practitioner.

Requirements

• Active Nurse Practitioner license with Family Nurse Practitioner (FNP) certification.
• Enrollment in Medicare and Medicaid programs.
• Strong communication and interpersonal skills.
• Ability to work independently and manage your schedule effectively.
• Prior experience in home healthcare or telehealth is a plus.

Nice to Have

• Experience with electronic health records (EHR) systems.
• Knowledge of value-based care models.
• Certification in advanced cardiac life support (ACLS).
