Remote Case/​Care Manager - Make a Difference in Healthcare

About the Role

We're hiring a Remote Case/Care Manager to join our dedicated team at Care Around the Block. This role is perfect for Registered Nurses (RNs), Licensed Practical Nurses (LPNs), or Social Workers who are passionate about making a difference in the lives of others. As a Remote Case/Care Manager, you will have the opportunity to provide individualized care services and support clients in navigating the healthcare community.

What You'll Do

• Conduct assessments and develop care plans tailored to client needs.
• Coordinate care and resources to support clients in achieving their goals.
• Work collaboratively with families, caregivers, and healthcare providers.
• Monitor progress and provide updates to ensure clients receive exceptional care.
• Develop and maintain an accurate and thorough knowledge of all program requirements.
• Comply with HIPAA and agency confidentiality policies.
• Complete and attest to the accuracy of payroll records.
• Maintain a professional demeanor with a positive approach to enhance engagement rates.
• Strong communicator, able to write clear documentation within 24 hours.
• Be self-directed, organized, and a team player.

Requirements

• Degree in social work, RN, or LPN preferred.
• Experience with community resources related to social determinants of health.
• Remote abilities with high-speed internet access.
• In-person assessments required monthly.
• Computer proficiency in Microsoft Word, Excel, and Outlook.
• Ability to pass a Criminal Background Check, Drug Screen, and DMV Check.

Nice to Have

• Previous experience in case management or care coordination.
• Knowledge of healthcare regulations and compliance.
• Familiarity with electronic health records (EHR) systems.
