Patient Care Coordinator - Remote Position

About the Role

We are looking for a dedicated Patient Care Coordinator to join our team remotely. As a Patient Care Coordinator, you will play a crucial role in managing in-home and virtual appointments for our healthcare providers and clinical staff. This position is essential in ensuring that patients receive timely and appropriate care, which is at the heart of ConcertoCare's mission. Your efforts will directly contribute to improving clinical outcomes for patients with complex needs.

What You'll Do

• Coordinate and schedule appointments for providers and clinical staff, both in-person and virtual visits.
• Utilize scheduling software to efficiently manage schedules and optimize provider productivity.
• Collaborate with operational and clinical teams to address scheduling needs and ensure alignment with organizational goals.
• Maintain accurate records of scheduled visits and cancellations to ensure compliance with healthcare laws and regulations.
• Communicate with patients regarding appointment confirmations and rescheduling, ensuring a positive patient experience.
• Participate in team meetings and huddles to discuss scheduling challenges and improvements.
• Address patient scheduling grievances promptly to enhance service quality.
• Other duties as assigned to support the overall mission of ConcertoCare.

Requirements

• High school diploma or GED.
• 2+ years of work experience in a healthcare setting.
• Preferred: Call center or scheduling experience.
• Preferred: Experience with patient communication protocols.
• Preferred: Experience in a value-based healthcare setting.
