Care Manager - Bilingual Spanish or Mandarin (Remote)

About the Role

We are looking for a dedicated Care Manager to join our team at The Child Center of NY. This is a remote position where you will play a crucial role in strengthening children and families by providing essential support and guidance. As a Care Manager, you will be responsible for participating in outreach and enrollment, assisting individuals, family members, and caregivers in accessing available services. Your work will directly impact the lives of over 40,000 children and their families each year.

What You'll Do

  • Conduct outreach and recruitment efforts to ensure accurate placement of eligible individuals and families.
  • Complete intake, enrollment, and ongoing assessments for enrolled members.
  • Facilitate interdisciplinary collaboration among providers, families, and caregivers.
  • Ensure families are aware of all support services and entitlements available to them.
  • Advocate for individuals and families by serving as a liaison between them and other programs.
  • Conduct office, home, and community visits to assess family needs and service delivery.
  • Assist in coordinating services for special needs, providing necessary support to families with children with disabilities.
  • Monitor compliance with all Federal, State, and Local regulations using data management systems.

Requirements

  • Bachelor’s degree in Human Services or a related field.
  • At least 2 years of experience in Human Services, Psychology, or a related field.
  • Coursework in social work principles, child development, counseling, or psychology.
  • Ability to plan and carry out assignments independently.
  • Strong prioritization skills and ability to multi-task.
  • Home visits are required in the Queens or Long Island area.
