Aged Care Comprehensive Assessor - Remote Opportunity
About the Role
Join our growing team as an Aged Care Comprehensive Assessor, working remotely while making a significant impact in the lives of older Australians. In this role, you will conduct clinical assessments from the comfort of your home and travel to various locations in the Paynesville, Bairnsdale, Lakes Entrance, and Bruthen Regions of VIC. As an Aged Care Comprehensive Assessor, you will connect clients with essential services that enhance their quality of life.
What You'll Do
- Conduct high-quality Aged Care Assessments to identify the needs of older adults.
- Facilitate referrals to essential services that support independent living.
- Promote functionality and capability in daily living for clients.
- Engage with families and carers, recognizing their role as key stakeholders.
- Ensure all assessments comply with Department of Health contractual requirements.
Requirements
- At least 1 year of experience in aged care, including client assessments and/or service delivery.
- Tertiary education in a health-related discipline (e.g., nursing, occupational therapy, physiotherapy, social work).
- Current unrestricted General Registration with AHPRA or relevant professional association.
- Proficiency in Word, Excel, Outlook, and client management systems.
- Current Australian driver license and a comprehensively insured vehicle.
- Permanent Resident or Citizen due to contractual requirements.
